Aflac Hospital Confinement

Aflac Hospital Confinement, also known as Aflac Choice, is available to employees and/or covered spouse and dependent children. It pays hospital related benefits to help with expenses not covered by major medical.

Plan Highlights:  

• Coverage available for employees and/or covered spouse and dependent children.
• Daily Hospital Confinement Benefit pays $50 per day per covered person for up to 365 days.
• Hospital admission – you choose $1,000 or $2,000 benefit amount at time of application which is payable once per year, per period of hospital confinement.
• Hospital emergency room - $150 for treatment in hospital ER. Limit 2 payments per year, per person.
• Hospital Short Stay – pays $100 for hospital stays of less than 23 hours, 2 payments per year, per policy.
• Extended benefit rider option available at time of application – pays benefits for visits to a physician, psychologist or urgent care center, lab test and x-rays, diagnostic/imaging exams and ambulance. These benefit amounts and limitations vary and will be provided to you at the time of application.
• Guaranteed Issue for basic coverage and extended benefit rider.
• Hospital stay and surgical care rider option available at time of application – provides benefits of $50 - $1,000 for a covered surgery, as well as benefits for invasive diagnostic exams, ICU confinement, and additional hospital confinement benefits. These benefit amounts and limitations vary and will be provided to you at the time of application, (medical questions are required for this rider).
